What to include in a correction

1. The exact URL and quoted statement.
2. Why the statement is incorrect, incomplete, outdated, or misleading.
3. The best primary source and publication or effective date.
4. Whether the error changes a broader conclusion.
5. Your name and organization, when relevant to the correction.

ICHRA Report may ask follow-up questions, preserve the original evidence record, or decline a requested change when the submitted evidence does not support it.

Provider updates and source submissions

Once submissions open, providers may send current product documentation, pricing schedules, implementation materials, service levels, security information, integration catalogs, outcome methods, and correction requests. Confidential material will not automatically be quoted or published. Information that cannot be verified publicly may be useful for context but may not support a public conclusion.
